NOTICE OF DECISION
Conditional Use Permit from Mountain Ministries Ranch
To Use the Former Middle School at 604 East "E" Street
To Operate a Church and Women and Children's Life Skills Training Center

Notice is hereby given that at the February 23, 2010 Public Hearing, the Rainier City Planning Commission approved a Conditional Use Permit request by Mountain Ministries to use the former middle school at 604 East "E" Street, Rainier, Oregon 97048 (Tax Lot 7216-044-03300) to operate a church and women and children's life skills training center.
Based on the findings of fact in the Staff Report dated 2-17-2010, applicant, citizen and agency comments on February 23, 2010 the Rainier Planning Commission approved the Mountain Ministries Conditional Use Permit request subject to the following conditions of approval:
That the number of trainees' & staff not exceed thirty (30), without prior written approval from the City of Rainier Planning Commission, as per "Finding # IV — Modifications".
That the duration of a trainees' stay in the Rainier Women and Children's Life Skills Training Center not exceed 24 months, without prior written consent of the Rainier City Planning Commission, as per "Finding # IV — Modifications".
All workers placing motor homes on the site will submit proper identification, approximate length of stay and any required vehicle license numbers to the Rainier Police Department and the City Planning Commission Clerks' Office, prior to placement of said units.
That the applicant obtain building "classification" from the Columbia County Building Department and present said classification notice to the Columbia River Fire & Rescue

Department and the City of Rainier Planning Commission Clerks' Office, prior to obtaining construction or remodeling building permits.
That all emergency ingress and egress routs be marked and approved by the City of Rainier Police Department; Public Works Department and the Columbia River Fire and Rescue Department, prior to obtaining an occupancy permit.
That the applicant apply for and obtain a "change—of—occupancy permit" from the Columbia County Building Department, as set forth in "Finding —111-B, steps 1-12" in this report, and submit copies of the "change —of-occupancy permit" to the Rainer Public Works; Columbia River Fire & Rescue Department and the City of Rainier Planning Commission Clerks' Office, prior to obtaining any construction building permits.
The applicant contact the Rainier Public Works Department and receive a statement of serviceability on all infrastructure (including site access drives) that service this site and submit a copy of the statement to the Columbia County Building Department, Columbia River Fire & Rescue Department and the City of Rainier Planning Clerks' Office, prior to obtaining an occupancy permit.
All electricians, plumbers, pipe fitters and general contractors working on site must be licensed by the State of Oregon. The name, address and license # of each contractor or sub-contractor must be submitted to the City of Rainier Planning Commission Clerks' Office and posted on the site in clear view, prior to commencement of work.
That prior to the issuance of the occupancy permit, a copy of the Oregon Child Care Division's (OCCD) "Certification Permit" issued by the Child Care Division 350, under Oregon Statute (ORS) 657A .260- Section 414-350 be submitted to the City of Rainier Planning Clerks' Office.
That prior to obtaining an occupancy permit, all fees related to the Conditional Use Permit Application review and public hearing process be paid in full.
That prior to placement of any identification or advertisement signs on the building or on the site, a sign permit application form be obtained, completed and proposal be approved by the City.
Appeal: A decision of the Planning Commission may be appealed only if, within fourteen (14) days after written notice of the decision is approved to the parties:
A party files an appeal petition and pays the required fee; or
The City Council directs that an appeal be initiated.

The City Council shall consider appeals of Planning Commission decisions by conducting an appeal hearing on the record. Only issues in the record before the Planning Commission, whether raised in writing or orally, shall be raised before the City Council. Notice of the appeal hearing shall be provided in the same manner and to the same parties as for the initial hearing. Notice of the City Council decision on the appeal, describing further appeal options, shall be provided to all parties to the proceeding within seven (7) days of the decision. The decision of the City Council shall be final unless an appeal is filed with the Land Use Board of Appeals within twenty-one (21) calendar days of the notice of the decision.
